Geriatrics


The Division of Geriatrics is dedicated to maintaining the well-being and vitality of older adults. Our goal is to help the aging live up to their full potential.

We interact with caregivers and family members on a personal level, as well as a clinical level, as we all work together to provide a consistently high level of patient care.

If a patient's condition requires an increase in care from an in-home setting to a hospital or nursing home, we maintain the continuity of medical attention for each situation.

We are one of the only geriatric group practices serving the residents of New York City's Upper West Side.

Disorders and diseases that we help our patients and their families manage
Our patients benefit from the skill of three geriatric physicians and one nurse practitioner who provide expertise in caring for the frail and elderly. We understand the potentially wide-ranging effects of geriatric illnesses.

We often work with patients and caregivers as a single unit, and are experienced in treating virtually all geriatric illnesses and disabilities including:
     • Cardiovascular conditions
     • Diabetes
     • Dementia and memory loss
     • Multiple medical problems
     • Osteoporosis
     • Pulmonary disorders
     • Thyroid disorders
     • Wound care
 

Services we provide for patients and their families

Comprehensive geriatric assessment
By conducting a comprehensive geriatric assessment, we can evaluate each patient's level of health, and determine their need for
     • Specific treatments or early intervention for certain illnesses
     • Further assessment by a neurologist or other specialist
     • More comprehensive or specialized care beyond home care, especially for
       patients experiencing severe memory loss or dementia

Home visitation
When a patient's condition becomes more impaired and prevents travel to our offices, we maintain care through our home visitation program.

We personally administer this program. Our own doctors and our nurse practitioner visit and care for our homebound patients.

Social services
The practice's own social worker assists patients and their families in identifying community resources, and helps them arrange for benefits or otherwise receive assistance.

Osteoporosis screening
We offer our patients free osteoporosis screening.

Research is an important element of improving patient care
Members of the Division of Geriatrics are also on the faculty of Columbia University College of Physicians and Surgeons, and members of St. Luke's-Roosevelt Hospital Center's Department of Medicine. We are committed to participating in leading-edge research involving both institutions.

Currently, division members are participating in a Medicare-coordinated care demonstration project.
