Diabetes Education Program

 

CLASS FORMAT

These are interactive classes.  Different tools will be used including “the Conversation Maps” from Healthy i, hands-on demonstration and illustrations.  We welcome questions and discussion. 

The instructor

A Health care Professional who is also a Certified Diabetes Educator will moderate the sessions according to the American Diabetes Association guidelines.

Should I come?

These classes are for any person with Type 1 or Type 2 Diabetes, controlled or uncontrolled, newly diagnosed or not.

Why should I come?

To better understand the many aspects of diabetes and to learn management skills that will help me better control my diabetes.

What Will I learn?

The facts and myths of diabetes, the importance of monitoring my blood glucose, and the interaction of food, exercise and medications will be taught. Other self-management skills will be discussed.

What should I bring to the classes?

Bring your testing supplies, your logs (if any) and your medications to every class.

 

 

Diabetes -- Diabetic patients have too much sugar in their blood. The disease can cause blindness, kidney failure, or result in amputation. Precise blood sugar control helps prevent serious eye, kidney, nerve and circulatory system damage.

We help our patients manage their diabetes and control blood sugar with diet and medications, including insulin. We also help them monitor their blood sugar and prevent other health problems.
